The Initial Care Coordination Call is a dedicated, in-depth conversation via video-call designed to thoughtfully plan care and ensure the right support is in place from the very beginning.
This call allows us to move beyond introductions and work together to create a personalized care plan that reflects your needs, routines, preferences, and goals.
During this call, we will:
• Review care needs in greater detail
• Discuss daily routines, preferences, and priorities
• Identify safety considerations and areas of support
• Determine recommended services and scheduling options
• Explain caregiver matching and next steps
• Answer questions from both clients and family members
This process helps ensure care begins smoothly, with clarity, consistency, and confidence for everyone involved.
